اثربخشی درمان مبتنی بر ذهن آگاهی و تغییر بر تکانشگری و رفتارهای خطرپذیری در نوجوانان با اختلال نارسایی توجه - فزون کنشی

زمینه و هدف: اگرچه برای بهبود نشانه ­های مرضی اختلال نارسایی توجه-فزون­ کنشی مداخلاتی طراحی شده است، با وجود این برای کاهش تکانشگری و رفتارهای خطرپذیری، اقدامات درمانی امیدوارکننده­ای انجام نشده است. در همین زمینه این پژوهش با هدف تعیین اثربخشی یک مداخله مبتنی بر ذهن­آگاهی که اخیراً برای بهبود تکانشگری و رفتارهای خطرپذیری در نوجوانان با اختلال نارسایی توجه- فزون­کنشی انجام شد.
روش: این مطالعه از نوع آزمایشی تک ­بررسی با خطوط پایه چندگانه و دوره پیگیری است. نمونه مورد مطالعه شامل 4 نوجوان با اختلال نارسایی توجه- فزون­ کنشی در دامنه سنی 15 الی 18 سال بود که از یک مرکز درمانی جامع در شهر رشت در سال 1399-1400 با روش نمونه­ گیری تصادفی ساده انتخاب شدند. برای اندازه­ گیری متغیرهای پژوهش از مقیاس تکانشگری بارت (پاتن و همکاران، 1995) و مقیاس خطرپذیری نوجوانان ایرانی (زاده محمدی و همکاران،1390) استفاده شد. درمان مبتنی بر ذهن­ آگاهی و تغییر در قالب 12 جلسه هفتگی 45 دقیقه­ ای اجرا شده و هر جلسه شامل یک تمرین ذهن­آگاهی 3 تا 16 دقیقه ای و نیز مرور تمرین خانگی و ارائه تکلیف خانگی جدید بود. از اندازه اثر، تغییر پایا، و درصد داده­ های غیرهمپوش برای ارزیابی معناداری آماری؛ و از درصد بهبودی برای ارزیابی معناداری بالینی استفاده شد.
یافته ­ها: یافته­ ها نشان داد که بر طبق سه شاخص تغییر پایا، اندازه اثر، و درصد داده ­های غیرهمپوش، اثربخشی این مداخله در کاهش تکانشگری و خطرپذیری از نظر آماری معنادار بود، اما شاخص درصد بهبودی نتوانست معناداری بالینی آن را نشان دهد.
نتیجه­ گیری: بر اساس یافته ­های به ­دست آمده می ­توان نتیجه ­گیری کرد که درمان مبتنی بر ذهن ­آگاهی و تغییر با تأثیر گذاشتن بر آگاهی از تجربه حال حاضر و تقویت مدیریت هیجان­ ها، قابلیت آن را دارد که تکانشگری و رفتارهای خطرپذیری را در نوجوانان با اختلال نارسایی توجه-فزون­ کنشی کاهش دهد.
