Developing a structural model of internalizing and externalizing behavioral problems in children with attention deficit- hyperactivity disorder based on attachment styles with the mediating role of typical family behaviors

Background and Purpose: Children with Attention-Deficit Hyperactivity Disorder (ADHD) profoundly affect the family functioning and psychological system. Therefore, this study aimed to develop a structural model of internalizing and externalizing behavioral problems in parents with ADHD based on attachment styles, with the mediating role of family typical behaviors.
Method: This study employed a descriptive-correlational design using structural equation modeling analysis. The statistical population included all parents of children with ADHD referred to psychological service centers in District 1 of Tehran in 2023. A sample of 276 participants was selected using convenience sampling. The research instruments included the Revised Adult Attachment Scale (RAAS; Collins & Read, 1990), the Child Behavior Checklist (CBCL; Achenbach, 1991), and the Family Time and Routines Index (FTRI; Cubin & Thompson, 1987). Data were analyzed using SEM in SPSS-27 and SmartPLS-4.
Results: Results showed that the path between anxious attachment, mediated by family typical behaviors, and internalizing problems (r = 0.096, p = 0.004) as well as externalizing problems (r = 0.059, p = 0.005) was positive and significant. Conversely, the path between secure attachment, mediated by family typical behaviors, and internalizing problems (r = -0.081, p = 0.005) as well as externalizing problems (r = -0.050, p = 0.016) was negative and significant at the 0.05 level.
Conclusion: The findings suggest that reducing anxious attachment, enhancing secure attachment, and improving family typical behaviors can decrease internalizing and externalizing behavioral problems. The role of secure attachment in promoting family typical behaviors and the detrimental effect of insecure attachment highlight the importance of considering attachment styles in this research.
